Taking both spironolactone and losartan together can cause an increase in potassium levels.

Char asked

Can spironolactone be taken with Hctz and losartan? Will potassium levels elevate too much?

At a glance

  • Spironolactone and Cozaar (losartan) both increase potassium levels (i.e. hyperkalemia). The use of them together increases this risk.

Answer

There is an interaction between these two drugs.

Taking both spironolactone and Cozaar (losartan) together increases the risk of hyperkalemia (high potassium levels).

Symptoms of high potassium levels include:

  • Muscle weakness
  • Abnormal heart rhythm
  • Chest pain
  • Palpitations

Spironolactone is known as a 'potassium-sparing diuretic', which, in general, are recommended to be used cautiously in those taking additional drugs that may increase serum potassium levels (like losartan).

It is important to note that although there is the potential for increased potassium levels, losartan and spironolactone are often prescribed together. However, be aware that your prescriber may order a blood test to check the amount of potassium in your blood while you are on the medications.

Lastly, be sure to avoid taking potassium supplements while taking these drugs unless they are recommended by your prescriber.

Below is some additional information regarding losartan and spironolactone.

Cozaar Info

Losartan is classified as an ARB (angiotensin II receptor blocker) and is used to treat high blood pressure and to reduce the risk of stroke in certain patients. It also slows the progression of kidney disease in patients with diabetes.

ARBs, like losartan, work on the same pathway as ACE-Inhibitors (renin-angiotensin system) but are associated with a lower incidence of cough, rash and taste disturbances.

Spironolactone Info

Spironolactone is classified as a 'potassium-sparing' diuretic. It helps you make more urine and to lose excess water from your body. It is used to treat high blood pressure, and edema or swelling from heart, kidney, or liver disease.

Spironolactone is also used to treat patients who make too much aldosterone or have low potassium levels.

Additional Information

Hydrochlorothiazide, commonly known as HCTZ (which is available with spironolactone in a single product, Aldactazide), does not interact with losartan.

In fact, it may help offset a potential increase in potassium as hydrochlorothiazide is a diuretic that can cause potassium loss
