Sue asked

Don't know if I should get my 2nd Shingrix vaccine because my arm was sore, red, achy and swollen for 10 days after the first vaccine. Had almost a "scar" 3" x 3" at the site after the redness went away! Thanks for your input!

Answer

Shingrix, the recently approved vaccine for shingle, can cause swelling at the injection site, sometimes severe.

According to the prescribing information for the drug, the incidence of swelling after administration is high, with around 26% of individuals experiencing it. However, the prescribing information specifically points out different severities of swelling. Around 1.1% of individuals experience more extensive swelling, which is defined as swelling covering an area of more than 100mm (almost 4 inches).

Having a more severe swelling reaction to the first dose of Shingrix is not a contraindication for the second dose, which you are scheduled to have between 2 to 6 months later. The only listed contraindication to Shingrix is a history of severe allergic reaction (e.g., anaphylaxis) to any component of the vaccine or after a previous dose of Shingrix.

It is important to discuss the side effect you experienced with your doctor so they can assess whether or not you have an allergy to the vaccine or if you may be susceptible to severe side effects with your next dose. In most cases, if you are not allergic to any component of the vaccine and your reaction was limited to local swelling, you will still be recommended to receive the second dose.
