Lady Di asked

Is it more beneficial to take Lisinopril-HCTZ in the morning or at dinner time?

Answer

It makes no clinical difference whether or not to take lisinopril-HCTZ in the morning or in the evening. However, any potential side effects you experience may dictate which time is best for you. In general though, it is more important that you take it at a time that is easy to remember so you don’t miss doses.

Lisinopril-HCTZ Dosing

Lisinopril-HCTZ is a combination of two drugs that work in different ways to lower blood pressure.

Lisinopril works by blocking the formation of angiotensin, which is a natural compound in the body that raises your blood pressure.  It is a very effective medication and is typically well tolerated. It may however, cause you to be slightly dizzy, tired or lightheaded. If any of these occur, it may be more beneficial to take the medication at night so these side effects aren't as bothersome.

In addition, lisinopril can sometimes cause an intermittent, dry cough. This is not a serious side effects and is reversible upon discontinuation. If the medication is causing a cough, be sure to speak with your doctor.

HCTZ (hydrochlorothiazide) works in the kidneys and helps the kidneys get rid of more water (you may have heard it be called a water pill). It may work to decrease blood pressure in other, less known ways as well.

As HCTZ makes you urinate more, it might cause you to wake up more often during the night to use the restroom if you take it in the evening.  If this is the case, it may be beneficial for you to take the medication in the morning.

Blood Pressure Medication Timing

There are a few studies that suggest it may be more beneficial to take blood pressure medication at night rather than the morning, but most studies show it doesn't matter.  More studies need to be done to really answer this question.

What is known is that having high blood pressure leads to many serious health problems like heart disease, heart failure, stroke, heart attacks, etc., so it is very important to take your blood pressure medications every day to keep your blood pressure under control.

Summary

As data regarding morning vs. evening dosing of blood pressure medication is inconclusive, the best time to take your lisinopril-HCTZ is:

  • When you can remember to take it every day.
  • When side effects are lessened (e.g. from excess urination or dizziness).
